Immunization Requirements
All Grades K-12
- Four doses of tetanus (one dose after the fouth birthday); three doses if series started after age seven
- Four doses of diptheria (one dose after the fourth birthday); three doses if series started after age seven
- Three doses of polio
- Two doses of measles
- Two doses of mumps
- One dose of rubella
- Three doses of hepatitis B
- Two doses of varicella or written statement from physician/designee indicating month and year of disease or serologic proof of immunity
Grades 7-12 (in addition to the above vaccines)
- One dose of tetanus/diptheria/pertussis (Tdap)
- One dose of meningitis vaccine (MCV4)
