1095-C Forms

Pursuant to the recently adopted Paperwork Burden Reduction Act of 2024 and beginning with the 2025 reporting year, the Moon Area School District will only send 1095-C forms to an employee if the employee makes a request for the form.  Please note that the 1095-C form is not necessary for an individual to file their tax return.

For reference, the 1095-C form lists each member’s name, address, date of birth, Social Security Number, and months of coverage during the year.

Requests will be fulfilled within 30 days of receipt in accordance with the Act.  Please ensure that your request includes the employee’s first and last name, current address, and the school entity the coverage was under during the 2025 calendar year.
